Multicultural Home Care, a Modivcare Personal Care Service, is looking for a Per Diem Nurse as a Field Supervisor responsible for ensuring quality and safe delivery of home health Care services such as supervising Caregivers/Certified Home Health Aides in a patient's home, evaluate the patient's plan of care and assess their physical and Psychosocial needs.

--- Schedule: Flexible, will do home visits when aides in service.
--- Pay rate per visit:

--- HHA Open $60
--- PC Intro $50
--- HM Open $40
--- Quarterly Supervision: $40

--- Visiting Client Territory: Brighton and surrounding areas

You will...

--- Performs on-site supervisory and teaching visits to evaluate caregivers
--- Provides care orientation to caregivers on assigned home care cases.
--- Participates in the training of new nurses and in-services training for field staff.
--- Performs initial nursing assessments
--- Developing a plan of care and makes necessary revisions based upon patient's condition/needs.
--- Document notes ensuring any changes in the service or patient's condition are reported
--- Monitors records for complete and accurate documentation.
--- Coordinates visits and communicate changes with the team
